• Provide, coordinate, and manage your healthcare and related services
• Bill and collect payment for services provided to you
• Comply with legal and regulatory requirements
• Assist with healthcare operations, such as quality assessment and improvement
   

You have the right to:

• Access and obtain a copy of your PHI
• Request corrections to your PHI if you believe it is incorrect or incomplete
• Request restrictions on certain uses or disclosures of your PHI
• Request confidential communications by alternative means or locations
• Receive an accounting of disclosures of your PHI
• File a complaint if you believe your privacy rights have been violated
   

• We are required by law to maintain the privacy of your PHI and provide this Notice of Privacy Practices.
• We will notify you if there is a breach of your unsecured PHI.
• We will abide by the terms of this Notice currently in effect.
   

We will not use or share your PHI for purposes other than those listed above without your written authorization. You may revoke your authorization at any time, except to the extent that we have already taken action based on your authorization.

You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, if you believe your privacy rights have been violated.
